*Currently we accept all Medicare B patients and we can provide you with a super-bill for submission of claims to your insurance provider for your out-of-network claims.

*Late cancellation fee of $ 45 will be charged to you, if no show or cancellation is requested within less than 24 hours of your appointment.
Office Ergonomic Consultation
1. Online Consultation (45 min) $150
2. Online Follow up visit (15-20 min) $50
